經常有人會說因為壓力太大而生病,到底這只是心理作用,還是長期承受壓力真的對身體有實質的影響呢?其實,壓力確實可以引發身體徵狀,亦可能影響我們的免疫力。1

壓力如何影響免疫系統?2,3
壓力可以對免疫系統產生正面和負面的影響,而當中分別就取決於壓力的持續時間:
- 短期壓力:會觸發「應戰或逃跑」的反應(Fight or flight response),增強免疫系統的準備,讓身體能夠應對當前的威脅
- 長期壓力:則會導致免疫抑制。長時間分泌壓力荷爾蒙如皮質醇,會破壞免疫功能,減少淋巴細胞和自然殺傷細胞的數量,並增加炎症

長期壓力與哪些健康問題有關?3-10
長期壓力與多種健康問題相關,包括:
- 心血管疾病:壓力會令心臟反應性增加(如心跳加快、血壓上升)、心臟血流量減少,以及皮質醇濃度提升。這些生理變化長期可能導致高血壓、動脈硬化、代謝性疾病及其他心血管疾病
- 消化系統問題:壓力可以引起腸易激綜合症、胃食道反流性疾病(亦稱「胃酸倒流」)等消化問題
- 慢性疼痛:壓力會加重偏頭痛和下背痛等慢性疼痛症狀
- 皮膚及頭髮問題:壓力可能導致痤瘡、牛皮癬及脫髮
- 二型糖尿病:長期壓力可能會令血糖持續偏高,增加患上二型糖尿病的風險
- 某些癌症:研究指出,長期壓力大可能提高癌症發病和死亡的風險

有研究指出,長期壓力會減少淋巴細胞和自然殺手細胞的數量、增加炎症,繼而削弱免疫系統,使其更容易感染傳染病,如感冒。

精神壓力影響身體的演變過程?11
科學家漢斯·薛利(Hans Selye)於1936年提出關於人體面對壓力時所經歷的三個反應階段,名為「一般適應綜合症」(General Adaptation Syndrome, GAS),當中包括警覺期、抗拒期和耗竭期。
- 警覺期(Alarm)
- 身體感知到壓力,釋放皮質醇、腎上腺素等激素,準備應對威脅
- 抗拒期(Resistance)
- 身體試圖恢復體內平衡,並維持代謝功能以保持正常運作
- 若壓力持續或增加,而身體無法恢復,便會進入第三階段
- 耗竭期(Exhaustion)
- 身體出現疲勞、倦怠、功能障礙、細胞與組織損傷及容易感染疾病等症狀

如何管理壓力以改善健康?2,13-15
想要減少長期壓力對身體健康帶來的負面影響,可參考以下策略來排解或管理壓力:
- 尋找壓力源:對壓力源有適當的認知會令排解壓力的整體難度下降,如工作上能制定時間表,安排工作順序
- 社會支持:與親友保持聯繫,尋求適當的支援能有效緩解壓力
- 正念及放鬆技巧:冥想、深呼吸練習等良好習慣可幫助降低壓力水平
- 適當運動:定期運動可以改善心理和身體健康
了解壓力對身體的影響十分重要,有助於我們更積極地管理壓力,維護整體健康!
